Milazzo is the main port, 45 minutes by car: the most frequent connections, hydrofoils and ferries all year. In summer there are also seasonal links from Sant'Agata di Militello and other harbours along the coast: convenient but with fewer runs, so always check the operator's current timetable before planning your day.
Which island with only one day
Lipari and Vulcano together are the classic combination: they're close, connections are frequent, and one day covers both. Salina deserves a full day to itself. Stromboli and Panarea as a day trip is a sprint: better one night on the island, especially if you want the evening volcano climb.
How to plan the day
First run of the morning; check the last useful return before you sail. In high season seats sell out: buy tickets online the day before. If you leave the car in Milazzo, budget for the paid car parks near the dock.
